REGISTRATION FORM - Notes

Notes to help with completion of the JPB Registration Form:

The form will check some entries on submission and you will be asked to correct or complete some entries if needed.

JPB Registration details: The first part of the form shows the information that you supplied at the time of registrations with the Joint Programme Board. The JPB number you have been allocated should be remembed and used when appropriate. If the email address has changed or is incorrect please correct both versions.

We will normally communicate with you via the email address that you supply.

Place of work: Your hospital where work on a daily basis. This should be in the list. If not please select "Not listed" at the end and enter your hopsital name.

Residential Adresses: This is the address where you normally live (sleep).

Please give any contact telephone numbers or an alterative email address if you have more than one.

Academic Centres: These should be listed. They have been defined by the JPB.

Students are registered with an academic centre and are allocated a tutor from an acredited training centre.

Training centres: The training centres have been agreed by the JPB and will be listed. In the case that your training centre is not listed enter the hospiatl name

Lead Tutor: The lead tutor for your hospital.

Practice Tutor:
Students are required to have a practice tutor who will be allocated to you by your base hospital / training centre.

It is of UTMOST importance that the information you supply about your tutor is accurate - especially email addresses. This will ensure that your tutor will receive all necessary information regarding the course, including details of tutor study days thus enabling your tutor to provide you with the support you require during the course.

Applicant Registration type and Fee options:

Standard entry applicant: For practitioners starting the programme from scratch.

(If you are unsure please check with the educational lead at your base hospital.)

Interim entry applicant: For pratitioners who have completed some form of pharmacy postgraduate study and wish to join the latter half of the programme.

(If you are usure please check with the educational lead at your base hospital.)

Fees Invoicing. Some hospital finance departments require Purchase Orders before invoice cans be raised. If your hospital is paying your fees please check if your hospital has such requirements and provide a purchase order number to facilitate invoicing. Please provide the address and other contact details of the finance officer for invioce purposes.

Statistical Data. We are required by the HEFC to collect statistics of registered students. This data will not not be used for any other purpose.
