Learning Set Swap Form

The purpose of this form is to request a temporary change of learning set for the requested set only. Please expect confirmation of the change. Without the confirmation your learning set has not been changed.

Ensure that you correctly indentify the learning set that you wish change and give the details for the new set.

Use the 'Submit' button at the end to submit your form. The form will automatically check some entries for validity before submission and request changes if errors are detected. You will be sent immediate confirmation by email that the submission was successful. Help notes for completion of form:

Important notes:

  • 1. If you cannot attend any other Learning Set or you do not know which Learning Set you are swapping to, please put NA in the boxes below and select ‘Cannot attend any other set’ in the drop down menu.
  • 2. If there is no Learning Set available in your intake you can attend the relevant Learning Set in 6 months time with another intake (you can find the list of all LS dates on ‘Current Practitioner’ page).
  • 3. If you have selected the ‘Cannot attend any other set’ option you must re-submit this form once the replacement Learning Set is known.
